This page is maintained by Indy Evals to answer common privacy questions about the Indy Evals website. This Privacy Policy covers information collected through the website only. Information you share as a client during therapy is protected health information (PHI) and is governed by HIPAA and our Notice of Privacy Practices.

Information we collect

  • Assessment / contact form: first name, last name, email address, phone number, appointment availability, and any short note you choose to share. This is used to respond to your request and schedule an intake.
  • Book Club sign-up: first name, last name, email address, phone number (optional), and the session you selected. This is stored so we can confirm your spot and send event details.
  • Automatic site data: basic IP address and browser information needed to deliver the site securely and prevent abuse. We do not use advertising or analytics cookies.

How we use it

We use the information you provide to respond to inquiries, schedule assessments, manage Book Club registrations, and keep the site secure. We do not sell, rent, or share your information for marketing.

How we protect it

The site is hosted on the Lovable Cloud platform, which provides standard encryption in transit (HTTPS) and database access controls. Website data is stored in our backend database and access is limited to Julie Otis, the practice owner. We do not make independent certification or audit claims on behalf of the platform.

Cookies and tracking

We do not use third-party analytics, advertising, or tracking cookies. We may use essential session cookies or storage for site operation, such as the admin login area and form state.

Data retention

We keep website contact/assessment requests and Book Club sign-up records for as long as needed to respond and provide services, and as long as required by HIPAA and Indiana professional standards. After that period, records are deleted or de-identified. You may request earlier deletion by contacting us.

Your choices

You can request a copy, correction, or deletion of your website-collected information at any time by emailing admin@indyevals.com. We may need to verify your identity before making changes.

Changes to this policy

We may update this Privacy Policy as the website or our practices change. The "Last updated" date at the top of the page will reflect the latest revision.
